From Fragmented Care to Designed Healing

The Community Integration Blueprint™
is Remedy & Root’s proprietary systems change engagement
for cancer programs and health systems. It’s a 90-day sprint to build patient navigation and community integration that pays for itself.

It’s the connective tissue that determines whether patients actually get what they need after they leave the exam room.

Why Now?

Since 2024, Medicare has paid for principal illness navigation and community health integration services. Yet, in a 2025 survey of 105 cancer care organizations, only 7% were implementing the codes. The barriers they named were unclear roles, staffing, documentation, and workflows. In other words, the infrastructure.

Meanwhile, navigators absorb duplicative, unbillable work, referrals leave your system’s walls and disappear, and the investments you’ve already made stay invisible in every board report.

The Blueprint builds the missing infrastructure.

What The Blueprint Helps You Do

  • Find the navigation work you're already doing and not capturing. Get a baseline of patients who are navigated but not billed.

  • Build workflows that meet billing requirements. Roles, documentation, and community partner agreements, designed with billing and compliance expertise.

  • Keep more patients in care. Close referral loops between clinic and community.

  • Meet the standards you're held to. This includes Commission on Cancer navigation expectations, community health needs assessment implementation, and value-based care measures.

  • Show your board the return. Population-level metrics tie community work to quality, retention, and cost.

How It Works: Four Phases in 90 Days

Diagnose (weeks 1 to 4): Scope, stakeholder list, and baseline review. The Systems Integration Readiness Assessment™, stakeholder and partner interviews, a community asset and barrier review, and a navigation and billing baseline.

Design (weeks 5 to 8): Stakeholder alignment on roles, ownership, and shared priorities. Workflows, partner agreements, and shared goals and metrics.

Activate (weeks 9 to 11): A pilot in one clinic or tumor site, with workflows live and measures being tracked.

Optimize (week 12): A final readout, written deliverables, and a plan to scale what works.
